This Pokemon Diamond game is actually a hack of the game Keitai Denjū Telefang  (Power Version). It comes with the box, instruction manual, and cartridge.
 
The box is in rough condition as can be seen in the photos. It also has a faint moldy smell to it.
 
Everything is tested and works fine.
